Information on how to refer families with children who have special health care needs to services
It takes a village to raise a child with special health care needs. According the American Academy of Pediatrics, there are many internal (emotional or behavioral) and external (interpersonal, housing, financial) risk and protective factors impacting wellness and resilience. When these factors are addressed in family centered, coordinated systems of care which are created in partnership with families, medical home/ primary care, mental and behavioral health providers, child care settings, social services, and other professionals, there is a positive influence on the health and wellness of the child and family resilience. Referring a family to the Bureau for Family Centered Services is simple: click to download the Bureau’s Application for All Services (Oficina De Servicios Centrados En La Familia (BFCS) Solicitud De Servicios) and then mail or email the completed form to the address at the end of the application. If you are unsure if services for children with special health care needs would be the right fit for a family, call the Bureau’s Coordinator on Duty.
