New Hampshire Feasibility Study for Programming to Promote Independence and Community Living for Dual-Eligible Individuals
Resources and meeting information for stakeholders about New Hampshire’s efforts to assess the feasibility of programs that promote independence and community living for dual-eligible individuals
DHHS is determining New Hampshire’s capacity to develop a Program of All-Inclusive Care for the Elderly (PACE) and/or a Dual-Eligible Special Needs Plan (D-SNP) program that promotes independence and community living for dual-eligible individuals. Over the next several months, DHHS is partnering with ATI Advisory, a research and advisory services firm, to conduct mixed methods analyses on the feasibility of launching PACE and/or D-SNP programs in New Hampshire.
As a part of the project, DHHS is soliciting input from stakeholders including dual-eligible individuals, advocacy organizations, community organizations and providers who work closely with these populations to understand barriers and facilitators to developing a program that will promote independence and community living for dual-eligible individuals.
