The Heart Disease Prevention Program is funded through grants from the Centers for Disease Control and Prevention (CDC) to:
Promote Technology for Patients
- Promote the adoption and use of electronic health records (EHR) and health information technology (HIT) to improve patient and provider outcomes
Sustain Community Health Workers
- Build a Statewide infrastructure to promote sustainability of Community Health Workers
Support Patients At Home
- Allow for more patients across the state to be able to monitor their blood pressure at home, with support from their primary care providers
